If you wish to register with the surgery please telephone 0131 554 1036 after 3pm on a Monday. One of our Patient Care Advisors will check that you live within our boundary area and will arrange to send you one of our Registration Packs.
Once completed, you can post this form into the General Correspondence post-box situated at the entrance to the surgery.
Our Registration Pack contains:
- Registration form.
- New patient questionnaire.
- Ethnicity form.
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP once registered as a temporary patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice. To register as a temporary patient please contact the surgery.
You cannot register as a temporary patient at a practice in the town or area where you are already registered.
